Insurance & reimbursement

EndoWell is a private, direct-pay practice and does not participate with insurance plans. Payment is collected at the time of booking to confirm your appointment.

Good Faith Estimate. Because EndoWell does not bill insurance, you have the right to receive a written Good Faith Estimate of expected charges. If your visit is scheduled 3 to 9 business days in advance, we will send the estimate within 1 business day of scheduling. If it is scheduled 10 or more business days in advance, we will send it within 3 business days. You may also ask for an estimate at any time, including before you schedule, and we will send it within 3 business days of your request. If your final bill is $400 or more above the estimate, you may have the right to dispute it through the federal payment dispute process. To learn more, visit www.cms.gov/nosurprises or call 1-800-985-3059.

Cancellations. Cancellations or reschedules made more than 48 hours before your appointment receive a full refund. Cancellations made between 24 and 48 hours before your appointment are charged 50% of the visit fee. Cancellations made less than 24 hours before your appointment, and missed appointments, are charged 100% of the visit fee. This policy applies to both in-person and virtual visits. As a courtesy and at EndoWell’s sole discretion, a charge may be refunded. For a patient who is a minor, the parent or legal guardian who books the visit is responsible for the charge. If your signed patient agreement differs from this summary, the signed agreement controls.

After each visit, we provide a superbill with the appropriate diagnostic and billing codes so you can request out-of-network reimbursement from your insurer, according to your plan’s benefits. When Dr. Luisa prescribes a medication or orders a procedure that requires it, our team will submit the prior authorization to your insurance. For laboratory testing and imaging, we include your insurance information on the order so it can be billed directly to your plan.

Coverage for medications, laboratory testing, imaging, and other services depends on your individual plan and the provider furnishing each service. EndoWell cannot guarantee coverage.

What to expect

Your consultation begins with the question, concern or change that brought you to EndoWell. We take the time to understand what you are noticing and review the relevant history and clinical context together.

If you have any prior labs, imaging, medications, diagnoses or treatment plans that may be relevant, you are welcome to bring them or send them in advance.
