Terms of use

Last updated: October 30, 2026   Effective date: October 30, 2026

Please read these Terms of Use carefully. By visiting or using endo-well.com (the “Site”), you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site. Nothing on the Site is medical advice, and using it does not make you a patient of the Practice.

1 · Who we are

The Site is operated by Luisa Gonzalez Ballesteros, M.D., PLLC, a New York professional service limited liability company (the “Practice,” “we,” or “us”). EndoWell is the brand name and logo used by the Practice on this Site and is not a separate legal entity. Our office is at 4 East 89th Street, Suite 1C, New York, NY 10128. Medical services are provided by Dr. Luisa Gonzalez Ballesteros (“Dr. Gonzalez Ballesteros”), a physician licensed in New York.

The Practice is independent. It is not part of, operated by, sponsored by, or speaking on behalf of any hospital, health system, medical school, or university with which Dr. Gonzalez Ballesteros is or has been affiliated. Content on the Site and on EndoWell social media accounts reflects the views of the Practice only.

2 · The Site is educational. It does not provide medical advice.

The text, video, images, downloadable resources, and other material on the Site (“Content”) are for general educational purposes only. Content is not a diagnosis, is not treatment, and is not a substitute for the advice of your own physician or other qualified health care professional. Never ignore professional medical advice, or delay seeking it, because of something you read or watched on the Site or on our social media accounts.

Reading or watching Content, sending an email or message, leaving a comment, or sending a direct message does not create a physician-patient relationship with the Practice or with Dr. Gonzalez Ballesteros. A physician-patient relationship begins only when the Practice has agreed to provide you or your child with care and you have completed the required intake and agreements. Content about medications, tests, or conditions is general and is not a recommendation for any individual. Medical knowledge changes, and Content may not reflect the most recent evidence.

3 · Emergencies

The Site, email, Klara patient messaging, and our social media accounts are not monitored for emergencies. If you or your child may be having a medical emergency, call 911 or go to the nearest emergency department right away. If you are thinking about harming yourself, call or text 988 (Suicide & Crisis Lifeline) in the United States.

4 · Contacting the Practice: use the approved secure channel for health information

To reach us, call the Practice or send us a message using the contact information on the Site.

  • Please do not send medical history, symptoms, test results, images, or other health information by email, by WhatsApp, through the contact form, in a social media comment, or in a direct message.
  • Email, WhatsApp, and social media messages are not intended for clinical information and may not provide the same privacy protections as the Practice’s secure clinical channels. For clinical information, the Practice will tell you which secure channel to use.
  • We do not guarantee a response time, and we do not answer individual medical questions through the Site, comments, or direct messages.

5 · Appointments, telehealth, and fees

The Site does not offer online booking. The Practice coordinates every appointment after you call or message us. No referral is required.

  • In-person care takes place at our New York City office. Families from other states and other countries are welcome to travel to New York for an in-person consultation.
  • Virtual visits are available, when telehealth is clinically appropriate, to patients who are physically located, at the time of the visit, in a state where Dr. Gonzalez Ballesteros holds an active medical license. We confirm the patient’s location before every telehealth visit. Patients outside the United States may request an educational consultation or second opinion; this does not establish ongoing treatment or include prescriptions. Patients under 18 must have a parent or legal guardian present.
  • Information on the Site about a service does not mean the service is available to you. Before scheduling, the Practice will tell you what it can and cannot offer in your situation.
  • Fees and insurance. The Practice does not accept or bill insurance plans, including Medicare and Medicaid. Patients pay the Practice directly, and payment is collected at the time of booking to confirm the appointment. Payment and cancellation terms are described on the Appointments page. Fees are stated in the Good Faith Estimate and in the patient agreements and consent forms you receive through Klara, our patient messaging platform, before your visit. If a summary on the Site differs from an agreement you have signed, the signed agreement controls. After each visit, we provide a superbill with the appropriate diagnostic and billing codes so you can request out-of-network reimbursement from your insurer, according to your plan’s benefits. Reimbursement is not guaranteed. When Dr. Gonzalez Ballesteros prescribes a medication or orders a procedure that requires it, our team will submit the prior authorization to your insurance. For laboratory testing and imaging, we include your insurance information on the order so it can be billed directly to your plan.
  • Good Faith Estimate. Because the Practice does not bill insurance, you have the right to receive a written Good Faith Estimate of expected charges. If your visit is scheduled 3 to 9 business days in advance, we will send the estimate within 1 business day of scheduling. If it is scheduled 10 or more business days in advance, we will send it within 3 business days. You may also ask for an estimate at any time, including before you schedule, and we will send it within 3 business days of your request. If your final bill is $400 or more above the estimate, you may have the right to dispute it through the federal payment dispute process. To learn more, visit www.cms.gov/nosurprises or call 1-800-985-3059.
  • Cancellations. Cancellations or reschedules made more than 48 hours before the appointment receive a full refund. Cancellations made between 24 and 48 hours before the appointment are charged 50% of the visit fee. Cancellations made less than 24 hours before the appointment, and missed appointments, are charged 100% of the visit fee. This policy applies to both in-person and virtual visits. As a courtesy and at the Practice’s sole discretion, a charge may be refunded. For a patient who is a minor, the parent or legal guardian who books the visit is responsible for the charge. If you have authorized a payment card on file in your patient agreement, the Practice may charge it for cancellation and missed appointment fees. If your signed patient agreement differs from this summary, the signed agreement controls.

6 · Social media, comments, and third-party platforms

The Practice shares educational content on third-party platforms such as Instagram, TikTok, Facebook, LinkedIn, and YouTube. Those platforms are governed by their own terms and privacy policies, and we do not control them.

  • Comments and public messages can be seen by others. Please do not post health information about yourself, your child, or anyone else.
  • Replying to a comment or message is not medical advice and does not create a physician-patient relationship.
  • We may hide or delete comments that contain personal health information, are abusive or discriminatory, are misleading, or are promotional.
  • We do not confirm or deny whether any person is or has been a patient of the Practice.

7 · Examples, stories, and results

The Practice does not publish patient testimonials, patient outcomes, or before-and-after content. Clinical situations described in Content are generalized and do not describe a specific patient, family, or visit unless we say so and have the required written authorization. When Dr. Gonzalez Ballesteros shares her own personal or professional experiences, she does so without identifying any patient. Nothing in the Content promises or guarantees any result. Google reviews that appear on the Site are shown automatically through Trustindex, were written independently by Google users, and are not edited by the Practice. They are not testimonials published by the Practice, they are not endorsements, and they do not promise or guarantee any result.

8 · Intellectual property and permitted use

The Content, the EndoWell name and logo, and related marks are owned by the Practice or its licensors and are protected by copyright, trademark, and other laws. All rights not expressly granted are reserved.

We grant you a limited, revocable, non-exclusive, non-transferable license to view the Site and to download or print single copies of Content for your personal, non-commercial use, provided you keep all copyright and other notices. Sharing a link to a page, or using the sharing tools we provide, is welcome. Without our prior written permission, you may not:

  • copy, republish, translate, modify, sell, or create derivative works from Content;
  • frame or mirror the Site, or present Content as your own;
  • use automated tools such as bots or scrapers to collect Content or data from the Site; or
  • use Content to train, fine-tune, or evaluate machine-learning or artificial intelligence models.

If you send us comments, ideas, or other material, you keep ownership of it, and you give us a non-exclusive license to use it to respond to you and to operate and improve the Site. Do not send material you do not have the right to share, and do not include health information.

9 · Acceptable use

You agree not to use the Site to break the law; to interfere with or disrupt the Site or its security; to attempt to gain unauthorized access to any system or account; to introduce malicious code; to impersonate any person or misrepresent your affiliation; or to harass others.

10 · Newsletter and downloadable resources

If you sign up for our email newsletter, we will send the general educational emails you agreed to receive. Downloading a resource does not, by itself, subscribe you to ongoing newsletters or marketing emails. We send those messages only when you separately sign up. You can unsubscribe using the link in any marketing email. We may change or withdraw resources at any time. See our Privacy Policy for how we handle your information.

11 · Third-party links and content

The Site may link to or embed third-party websites, videos, and services. We do not control them, do not endorse them unless we say so, and are not responsible for their content, accuracy, or privacy practices. Your use of them is at your own risk and subject to their terms. If a piece of Content is sponsored, or we have a financial relationship with a product or service it mentions, we will disclose that clearly.

12 · Visitors outside the United States

The Practice is based in New York and the Site is intended for a United States audience. Families outside the United States are welcome to contact us. The Practice offers educational consultations and second opinions to patients outside the United States. These do not establish ongoing treatment or include prescriptions. We will tell you what we can and cannot offer before any appointment is scheduled. If you access the Site from outside the United States, you do so on your own initiative and are responsible for compliance with local laws.

13 · Children

The Site is directed to adults, including parents and legal guardians. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you believe a child under 13 has provided personal information through the Site, contact us. We will take appropriate steps to delete it unless we are required or permitted by law to retain it. Care for children and adolescents is provided with the involvement of parents or guardians as described in the patient agreements and our Notice of Privacy Practices.

14 · Accessibility

We want the Site to be usable by everyone and we work toward conformance with the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA. If you have difficulty using any part of the Site, or need Content in another format, contact us using the details in Section 23 and we will work with you.

15 · Privacy

Our Privacy Policy explains how we handle information collected through the Site. Our Notice of Privacy Practices explains how we use and protect protected health information when HIPAA applies. Both are available from the footer of the Site.

16 · Disclaimer of warranties

TO THE FULLEST EXTENT PERMITTED BY LAW, THE SITE AND THE CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE CONTENT IS ACCURATE, COMPLETE, OR CURRENT, OR THAT THE SITE WILL BE UNINTERRUPTED, SECURE, OR FREE OF ERRORS OR HARMFUL COMPONENTS.

This disclaimer applies to the Site and the Content. It does not change the professional obligations that Dr. Gonzalez Ballesteros and the Practice owe to a patient once a physician-patient relationship exists. Those obligations are governed by applicable law and professional standards.

17 · Limitation of liability

TO THE FULLEST EXTENT PERMITTED BY LAW, THE PRACTICE, DR. GONZALEZ BALLESTEROS, AND THEIR MEMBERS, EMPLOYEES, AGENTS, LICENSORS, AND SUPPLIERS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS OR LOST DATA, ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE SITE OR THE CONTENT, OR FROM RELIANCE ON THE CONTENT. OUR TOTAL LIABILITY FOR ALL CLAIMS RELATING TO THE SITE WILL NOT EXCEED US$1,000.

Nothing in these Terms limits or excludes (a) liability for professional services provided to a patient in a physician-patient relationship, including any claim for professional negligence; (b) liability for gross negligence, fraud, or willful misconduct; or (c) any liability that cannot be limited or excluded under applicable law.

18 · Indemnification

To the extent permitted by law, you agree to defend and indemnify the Practice and Dr. Gonzalez Ballesteros against claims, losses, and reasonable legal fees arising from your violation of these Terms or your unlawful use of the Site. This section does not apply to a patient’s use of the Practice’s professional services.

19 · Governing law and venue

These Terms and any dispute arising from your use of the Site are governed by the laws of the State of New York, without regard to conflict-of-law rules. You and the Practice agree that the state and federal courts located in New York County, New York, have exclusive jurisdiction over any such dispute, and you consent to personal jurisdiction there. Disputes about medical services are governed by applicable law and by the agreements you sign as a patient, not by this Section.

20 · Copyright complaints

If you believe Content on the Site infringes your copyright, send a written notice to our designated contact: Luisa Gonzalez Ballesteros, M.D., PLLC, 4 East 89th Street, Suite 1C, New York, NY 10128, contact@endo-well.com, +1 (917) 573-0248. Please include: a description of the copyrighted work; the location (URL) of the material you believe is infringing; your contact information; a statement that you have a good-faith belief the use is not authorized by the owner, its agent, or the law; a statement, under penalty of perjury, that your notice is accurate and that you are the owner or authorized to act for the owner; and your physical or electronic signature. We may end access for repeat infringers in appropriate circumstances.

21 · Changes

We may update the Site and these Terms at any time. The “Last updated” date above shows when these Terms last changed. Your continued use of the Site after an update means you accept the updated Terms. We will make reasonable efforts to highlight material changes.

22 · General

If any part of these Terms is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver. You may not assign these Terms; we may assign them in connection with a change in the ownership of the Practice, subject to applicable law and our obligations to patients. These Terms and the Privacy Policy are the entire agreement between you and the Practice about use of the Site; they do not replace any agreement you sign as a patient. This document is provided in English and Spanish. If the two versions differ, the English version controls. Sections 7, 8, 16, 17, 18, and 19 remain in effect even after you stop using the Site.
